Nephrology – CKD with Chronic Hyponatremia
83F with PMHx HTN, DM2, breast ca, COPD NSTEMI, and acute/chronic diastolic CHF and is a resident. Her Port was infected. She was recently hospitalized for bacteremia. Blood culture with staph. Aureus. She received vancomycin, Ceftarolin, and Ancef. Her Mediport was removed. Upper extremity PICC line inserted for 6 weeks antibiotic. She was discharged on Ancef.
Nephrology – CKD w/CHF
76 y/o female with CKD, Heart failure seen due to increased weight 10 lbs in 3 months, 6 lbs in 2 months, and 4 lbs in 1 month, and BLE edema. She denies SOB, CP, and no orthopnea; Torsemide increased to 40mg 5 days a week from 20mg 4 days a week. Her kidney function is currently: Crt 2.73, BUn 64, and GFR 16. CL 113. No complaints. Continue to monitor. She remains DNR DNI DNH. The goal is comfort.

This female with active medical conditions: DM2, HTN, CKD4, dementia, GERD and OSA, is a 76 y/o AA s/p 12-day hospitalization from 5/28 through 6/9 for toxic encephalopathy and tremors. Work-ups while hospitalized included UA, CT head, VBG, TSH, ammonia, and B12, all of which were unremarkable. Her Zyprexa and Tramadol were discontinued, and weaning of Baclofen began with noted improvement.
