Patient analytics flags care gap
Activity in the patient’s chart reveals uncontolled high blood pressure. AristaMD’s care coordination team notifies the PCP and schedules the patient for a telehealth visit with a Cardiologist.
AristaMD specialist evaluates patient
Using our secure, HIPAA-compliant telehealth service, a board-certified cardiologist meets with the patient to discuss prescribing an additional medication to treat the patient’s high blood pressure. The specialist reviews the dosage instructions and at-home monitoring with the patient. She also recommends a follow-up visit with the patient’s PCP.
Care coordinator schedules follow up with PCP
A care coordinator contacts the patient to ensure the specialist’s treatment plan is followed and confirms the patient is monitoring his blood pressure daily. The patient’s follow-up visit with the PCP is also scheduled.
PCP remains at the helm of care
The PCP receives the evidence-based treatment plan from the AristaMD care support team and has access to the specialist if questions arise regarding the patient’s care.
